Prospective students should also be aware that most of the PADI courses have a swimming requirement (varies per course). Before signing up for a class, we encourage a discussion with the course instructor about the swimming requirement of that course. We strongly oppose mid-course dropouts because swimming was not addressed prospectively.

 

 

 

Training Administrative Requirements

Medical

We strongly encourage all our prospective students to first review the medical requirements for training and whether you will be required to secure a physician's statement prior to engaging in diving activities. By clicking on the Medical form link at left, you will be able to download the standard PADI medical form in PDF format. Note that any answer "yes" in the questionnaire will require you to secure a physician's signature on the form. Any physician signing the form positively must have an arms-length relationship with the potential student diver. The form has substantial medical information to assist your physician in making a determination. However, the physician only has to use his/her best judgment regarding the medical condition of the prospective student diver as it applies to diving activities and check the corresponding block over his/her signature. The final decision to accept or not accept a student for training always lies with your instructor.

Liability Release and Assumption of Risk

Prior to engaging in any form of training both PADI and Aqua Clara Dive Schools of Florida, Inc. require a prospective student diver to execute a standard PADI Liability Releases and Assumption of Risk form. Standard Safe Diving Practices

Also prior to engaging in any form of training both PADI and Aqua Clara Dive Schools of Florida, Inc. require a prospective student diver to execute a standard PADI Standard Safe Diving Practices Statement of Understanding form. By clicking on the Safe Diving form link at left, you will be able to download that form in PDF format.

[Note: legal guardian signatures are required for minors. Please contact us for additional information if the prospective student diver is under 18. All medical requirements remain the same.] 

Combination Form for Continuing Education

For those who anticipate taking more than a single course there is a Continuing Education Administrative Document which incorporates all three of the above forms and remains valid for one year. By executing this form, it precludes the prospective student diver from having to newly execute each of the above forms for each course they decide to take. Note, however, that any change in medical status during the course of the one year period may require a new physician's statement.  As mentioned above, the final decision to accept or not accept a student for training always lies with your instructor.
